Specialty Doctor - Geriatric Medicine (Acute Care at Home)

Geriatric Medicine locum doctor position at Southern Health and Social Care Trust, Northern Ireland — £60–£70/hr assignment rate.

Grayscale Medical is recruiting a Specialty Doctor (SAS grade) for a long-term locum assignment with the Acute Care at Home service within the Southern Health and Social Care Trust. This community-based role is part of the Adult Community Services directorate, delivering acute geriatric medicine care to patients in their own homes.

The assignment runs from October 2026 through to mid-January 2027, with daily daytime shifts (09:00–17:00) including weekends. You will provide senior clinical oversight within the Hospital at Home model, managing acutely unwell elderly patients, supporting admission avoidance, and facilitating early supported discharge. The caseload includes assessment and management of falls, delirium, acute infections, cardiac failure, COPD exacerbations, and palliative care needs in a community setting.

Candidates must be registered with the GMC with a licence to practise, at Specialty Doctor or Staff Grade level with substantive experience in Geriatric Medicine or General Medicine. Right to work in the UK, up-to-date DBS/AccessNI clearance, and professional indemnity insurance are required. Community medicine or hospital-at-home experience is highly desirable.
